If it were voluntary, which of these teams would choose to play in an elite, open division, where anywhere from half to all of the teams would compete for the ultimate state championship at the end of the season?

It would vary from year to year. If a team is a bit below their best years and is borderline on qualifying, they may sit out. If a team, no matter how strong, suffers a major injury or injuries late in the season, they may decline.
